In an effort to make premium remittance easier and more accurate for our Self-Administered (non-Web Billed) groups, the Self-Administered Premium Report is now available online. In addition to completing your group’s name and number, you can now fill in the number of lives and volumes from your previous statement. Your premium will automatically calculate, and you can print a copy and send it to us along with your payment.
Simply go to the Forms tab and select your state. Under the Group Administration section, select Self-Administered Premium Report. Instructions are also provided for your convenience.
